Normal capillary refill (pinching the nail bed until the skin blanches, then counting the seconds for the color to return) is <2 s. A capillary refill time of >2 s implies poor perfusion. Skin turgor is evaluated by pinching the skin and allowing it to return to its original state. Normally, the skin bounces back quickly. Tenting or decreased skin turgor implies malnutrition or dehydration. Other signs of dehydration include hypotension, tachycardia, and diminished peripheral pulses.
